Your United States Passport Acceptance Facility

Take advantage of the Buena Park Library District’s convenient passport services and help support library programs at the same time.

Important Passport Information: 

  • All passport applicants must apply in person, regardless of age.
     

  • When a child is under 16, both parents must be present and show identification when applying for their child’s passport. If one parent is unable to come, a notarized Statement of Consent from the absentee parent can be used. When a child is 16 - 17 years of age, at least one parent must be present and show identification when applying for their child's passport. Please see the U.S. Department of State website for further information.
     

  • You can expect your passport to arrive within 6-8 weeks or, for an additional $60, you may request expedited service to receive  your passport within 3 weeks.
     

  • If you are traveling within 14 days, you must call the automated appointment line at 1-877-487-2778 to request an appointment with the Los Angeles Passport Agency. You will need to show proof of travel.

  • U.S. Passport Card:

    U.S. citizens may begin applying in advance for the new U.S. Passport Card beginning February 1st, 2008, in anticipation of land border travel document requirements. We expect cards will be available and mailed to applicants in Spring 2008. The passport card will facilitate entry and expedite document processing at U.S. land and sea ports-of-entry when arriving from Canada, Mexico, the Caribbean and Bermuda. The card may not be used to travel by air. Please refer to the U.S. Department of State website for further information. 

You will need the following items:

  • Passport Photos
    You will need 2 identical color passport photos, 2” x 2” in size, taken within the past 6 months. Photos need to have a plain white background. You must be looking straight ahead. Do not wear anything that obscures the hair or hairline. Dark glasses or tinted lenses that obscure the eyes cannot be worn.

    Photo services are available at the Library during all Passport Acceptance Service Hours.

  • Passport Application Forms
    You will need the DS-11 form to submit your application. The forms are available at the Library, or you can print them from the Passport Agency website.  Be prepared to supply: your Social Security number, your parents’ birth information (even if deceased), and, if you have been married your date of marriage and your current and/or previous spouse’s birth information. Please do not sign the DS-11 form until the Passport Acceptance Agent instructs you to do so.

    If you have a passport that was issued less than 15 years ago and you were at least 16 years old when it was issued, you may be able to submit a self-service renewal.  Self renewal forms are available at the Buena Park Library or you can print them from the Passport Agency website. 
     

  • Proof of Citizenship
    You need either a certified copy of your Birth Certificate issued by the city, county or state of your birth OR a current or previous U.S. Passport OR an original Certificate of Naturalization. Hospital Birth Certificates and California or Texas Certified Birth Abstracts are NOT accepted for proof of citizenship. Helpful information about how to obtain a certified birth certificate is available on the Internet on the Where to Write for Vital Records webpage at the National Center for Health Statistics. Similar information can be found through the VitalChek Network Inc. website.

    The U.S. Department of State will determine the acceptability of all documents presented. Your original documents will be mailed with your passport application and will be returned with your new passport.
     

  • Proof of Identity
    As proof of identity adults may show one of the following: a previous U.S. passport, a Naturalization Certificate, a current valid U.S. driver’s license, a U.S. state issued identification card, a Military I.D. card, or Government (city, state or federal) issued photo I.D. card.
